1. Fill out the attached migration form with the following: Name, T-Mobile Billing Account Number, Billing Address, and Farm Bureau membership number. Under "Plan Migration Information" please be sure to include your T-Mobile phone number.

2.You do not need to provide a work or association number, nor do you need a company or association e-mail address.

3. Send the form to the email address on the form. Do not send the form to Aurline Boyack.

4. Please allow one to two billing cycles for the discount to appear on your account.

5. The member receiving the discount must be the T-Mobile account holder or listed as an authorized user on the account.

6. Rate plans including but not limited to Flex pay, Pre-paid, Unlimited Loyalty, Kid Connect and Even More Plus do not quality for the discount.

7. Upgrades on devices must be done through customer service.
